We are currently seeking a skilled and dedicated LPN Homecare Visit Nurse to join our team. In this role, you will have the opportunity to make a meaningful difference in the lives of our patients by providing comprehensive nursing care during home visits. Schedule Monday thru Friday (Hours may vary based on each case)

Responsibilities:

Conduct home visits to assess patients' health status and provide nursing care as prescribed by the healthcare team.
Administer medications, treatments, and procedures according to established protocols and physician orders.
Monitor patients' vital signs, symptoms, and progress, and report any changes or concerns to the appropriate healthcare professionals.
Educate patients and their families about their conditions, medications, and self-care techniques to promote optimal health outcomes.
Collaborate with other members of the healthcare team, including physicians, therapists, and social workers, to develop and implement individualized care plans.
Document all patient interactions, assessments, and interventions accurately and timely in the electronic medical record.
HHA Supervisory visits to monitor care.
Wound care

Qualifications:

Current LPN license in the state of Ohio .
Minimum of 1 years of nursing experience, preferably in a homecare or similar setting.
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal abilities.
Ability to work independently and as part of a collaborative team.
Compassionate and patient-centered approach to care.
Valid driver's license and reliable transportation.
One year of experience as a home care professional nurse and is competent in performing home care comprehensive assessment
