 Base $45,000-$60,000 plus commissions

Build and own referral relationships for a licensed private-duty home care agency in Southwest Atlanta and College Park. You report directly to the owner.

The agency
BrightStar Care of Southwest Atlanta & College Park is independently owned and operated under the national BrightStar Care brand. We opened in 2026 and serve 14 ZIP codes in Fulton County. Services include companion care, personal care, and skilled nursing, and every client receives an RN assessment. Clients pay privately or through long-term care insurance, VA Aid & Attendance, or Medicare Advantage supplemental benefits. We do not currently accept Medicaid.

The role of sales development / referral partnerships manager
Our referrals come from the people responsible for a patient's next step: discharge planners, case managers, DONs, physicians, assisted living administrators, and hospice and rehab staff. They are not buying anything from us, but do decide whether to trust us with a patient or family. You learn what each source needs, decide how we fit, and earn referrals over time.

What the sales development / referral partnerships manager will do
  • Meet in person with referral decision-makers across the territory
  • Research each source and plan your approach before you visit
  • Keep a tiered list of referral sources and track every contact in HubSpot
  • Host in-services and lunch-and-learns, and attend community events
  • Update referral partners after their clients start care
  • Work with our Director of Nursing so referrals turn into well-matched care
  • Review results with the owner weekly
What we expect of the sales development / referral partnerships manager
  • 40 in-person contacts per week with staff at referral sources, at least half with decision-makers
  • Time in the field, with occasional evenings or weekends for events
Who this fits
  • 1-3, or more, years of experience in hospice or home health community liaison work, SNF or ALF admissions or community relations, discharge planning, case management, social work, or a similar referral-side healthcare role
  • Experience building relationships with clinicians and administrators where the work was understanding their needs
  • Comfortable approaching clinicians and administrators cold, and following up consistently
  • HubSpot or similar CRM experience and Atlanta healthcare market knowledge are a plus
  • Reliable transportation and a valid driver's license
Compensation and benefits
  • Base salary of $45,000-$60,000, depending on experience
  • Commission based on revenue obtained, paid quarterly
  • Paid time off
  • Gas and travel reimbursement
Compensation: $45,000.00 - $60,000.00 per year

U.S. Equal Opportunity Employment Information (Completion is voluntary)

We are an equal opportunity employer and all qualified applicants will receive consideration for employment without regard to race, color, religion, sex, national origin, disability status, protected veteran status, or any other characteristic protected by law.

You are being given the opportunity to provide the following information in order to help us comply with federal and state Equal Employment Opportunity/Affirmative Action record keeping, reporting, and other legal requirements.

Completion of the form is entirely voluntary. Whatever your decision, it will not be considered in the hiring process or thereafter. Any information that you do provide will be recorded and maintained in a confidential file.

Form CC-305
OMB Control Number 1250-0005
Expires 4/30/2026

Why are you being asked to complete this form?

We are a federal contractor or subcontractor. The law requires us to provide equal employment opportunity to qualified people with disabilities. We have a goal of having at least 7% of our workers as people with disabilities. The law says we must measure our progress towards this goal. To do this, we must ask applicants and employees if they have a disability or have ever had one. People can become disabled, so we need to ask this question at least every five years.

Completing this form is voluntary, and we hope that you will choose to do so. Your answer is confidential. No one who makes hiring decisions will see it. Your decision to complete the form and your answer will not harm you in any way. If you want to learn more about the law or this form, visit the U.S. Department of Labor’s Office of Federal Contract Compliance Programs (OFCCP) website at www.dol.gov/ofccp.

If you believe you belong to any of the categories of protected veterans listed below, please indicate by making the appropriate selection. As a government contractor subject to Vietnam Era Veterans Readjustment Assistance Act (VEVRAA), we request this information in order to measure the effectiveness of the outreach and positive recruitment efforts we undertake pursuant to VEVRAA. Classification of protected categories is as follows:

A "disabled veteran" is one of the following: a veteran of the U.S. military, ground, naval or air service who is entitled to compensation (or who but for the receipt of military retired pay would be entitled to compensation) under laws administered by the Secretary of Veterans Affairs; or a person who was discharged or released from active duty because of a service-connected disability.

A "recently separated veteran" means any veteran during the three-year period beginning on the date of such veteran's discharge or release from active duty in the U.S. military, ground, naval, or air service.

An "active duty wartime or campaign badge veteran" means a veteran who served on active duty in the U.S. military, ground, naval or air service during a war, or in a campaign or expedition for which a campaign badge has been authorized under the laws administered by the Department of Defense.
