Licensed Practical Nurse (LPN) – Home Health

Salary: $35–$45 per hour
Bonuses: Referral Bonus + Performance Bonus

Join BrightStar Care as a Licensed Practical Nurse (LPN) in Waltham, MA, where you’ll deliver compassionate home health care directly to clients in their homes. Responsibilities:

Administer medications and treatments as prescribed by healthcare providers.
Monitor and document patient vital signs and overall health status.
Assist with daily living activities such as bathing, grooming, and meal prep.
Educate patients and their families on managing health conditions and following care plans.
Coordinate with RNs, therapists, physicians, and other healthcare professionals.
Maintain clear, accurate, and timely documentation using EHR systems.
Provide emotional support and companionship.
Follow all safety and health regulations in home care settings.

Requirements:

Active and valid LPN license in the state of Massachusetts.
Minimum 1 year of experience in home health or related healthcare setting.
Current BLS certification.
Reliable transportation and willingness to travel to patient homes.
Proficient in electronic health records (EHR).
Excellent communication and interpersonal skills.
Strong sense of compassion and dedication to quality patient care.
