Key Responsibilities:
  • Perform in-home PT evaluations and create personalized care plans
  • Provide skilled PT interventions to enhance strength, balance, mobility, and safety 
  • Educate clients and families on exercises and home safety
  • Collaborate with a multidisciplinary care team
  • Document care using mobile-friendly EMR
Requirements:
  • Active MA PT license
  • DPT or MPT from an accredited program
  • CPR certified
  • 1+ year of experience preferred (new grads welcome)
  • Reliable vehicle & valid driver’s license
