What You’ll Do

Perform PT evaluations and follow-up visits per physician orders
Develop and adjust individualized plans of care
Deliver therapy for gait, balance, mobility, strength, and fall prevention
Train patients and caregivers on home exercises and safe mobility strategies
Document per NJ DOH, Medicare, and BrightStar standards
Collaborate with RNs, OTs, STs, and CHHAs

What We Need

NJ Physical Therapy license in good standing
1+ year of PT experience (home health preferred)
CPR/BLS certification
Reliable transportation within Camden/Cherry Hill
Strong documentation and patient communication skills

Compensation: $65.00 per hour
